What is laser toning?
Laser toning is a comprehensive skin care tool that dermatologists and skin surgeons use to treat skin and hair conditions. The laser toning technique is applied to minimize the appearance of wrinkles, pigmentation, acne scars, fine lines, and active acne. This treatment works by accelerating the production of natural collagen deep within the skin. Best of all, the laser therapy does not cause any cut, burn, sensitivity or peeling of the surface layers of the skin. It smoothly works through the outer layers of the skin to accelerate collagen production in the underlying layers of the skin.
How the Skin Laser toning Works
The laser toning method is a far reaching process that penetrates its rays deeper into the skin and works by heating only the upper dermis, near to the surface of the skin, for the most noticeable results. The good part is that the patient does not feel this heat on the surface of their skin because of the laser’s in-built cooling system specifically designed to safeguard the skin from harmful UV rays damage. The laser device’s heat aims the dermis layer, which produces collagen naturally. As this part of the skin is accelerated, collagen production boosts up, minimizing the appearance of wrinkles, acne marks, pimples, scars, fine lines and the sebaceous glands are forced to contract. This also helps to keep control upon any active acne the patient may have.
Laser Toning Procedure
As of preparation, before beginning with the laser toning treatment, your dermatologist may apply lidocaine or some other local anesthetic to the skin. Topical local anesthetics usually take half an hour to come into effect.
Each laser toning treatment for improvement of in the skin imperfections like wrinkles, acne or scarring etc, takes about 15 to 30 minutes. Generally, multiple treatments are required to obtain the positively visible result. Laser when used for the treatment of active acne, it must be given regularly to the patient by the dermatologist to control the sebaceous glands.
Most pigmented skin lesions, whether dermal or epidermal, received or inherited, can be effectively treated with Q-switched lasers of green, blue, and infrared light.
What all are the applications of Laser toning?
There are numerous clinical applications of Laser toning, a few are listed below:
Melasma
Epidermal melasma is reportedly known to respond well and quicker than dermal or mixed melasma. Total clearing of lesions may be anticipated in more than 60% of cases for epidermal melasma. Total clearing of dermal or mixed melasma is observed in approximately 30-50% of cases, while the remaining cases will be seen with moderate improvement. Rebound melasma and Post inflammatory hyper pigmentation are frightful complexities that may occur in a person with super sensitive skin. Minimal laser repetitions and lower laser energy are suitable to provide marked improvement. However, improvement is required to be sustained by repeated sessions. However, recurrence is general in melasma.
High-frequency energy selective laser, for example, 532 nm frequencies doubled Q-switched NdYAG laser, 694 nm, Q-switched ruby laser, 755 nm Q-switched alexandrite laser, and 1064 nm Q-switched NdYAG laser had been researched for treatment of melasma with inadequate results. Normal skin color was barely obtained.
Pigmented Lesions
Dermal pigmented lesions
Mongolian spots and other flat pigmented birthmarks respond well to laser toning. The Nd:YAG lasers at 1064 nm of wavelength works best for dermal pigmented lesion conditions. There are multiple sessions generally needed with near-complete clearing of the lesion in most cases.
While the epidermal lesions counter well to 532 nm of wavelength (frequency half the NdYAG) the dermal lesions which are better treated with 1064 nm.
The Q-switched lasers are the gold excellence merit for treatment of tattoos.
Freckles
It responds well to laser treatment. However, quite effective, risk of dys-pigmentation prevails.
Lentigines
It generally requires just 1-2 sessions to clear lentigines wit laser at 532 nm of frequency. However, very effective, there is a risk of hyper pigmentation that necessitates avoidance of sun exposure for about 4-6 weeks post laser toning.
Cafe-au lait macules
Treatment of Cafe-au lait macules lasers is very effective and can be treated sufficiently in 1-2 sessions, but recurrence is reckoned which necessitates multiple treatments.
Tattoo Removal by Lasers
Although Q-switched alexandrite and Q-switched ruby lasers have been the extensively used lasers for tattoos since the earliest times, Q-switched NdYAG 1064 nm, due to its far reaching wavelength, higher fluence, and smaller pulse, has transpired to be as an ideal laser for the black and dark blue tattoo pigment. The evident changes in skin texture, hyper pigmentation and scarring as compared to earlier lasers are notably low. However for different colored pigments, use of varied wavelengths is important. Response of laser toning specifically with Q-switched 1064 nm depends on the type of tattoo:
Professional Tattoos
These types of tattoos mostly have even distribution of ink pigment, chiefly in subcutaneous tissue. In professional tattoos ink quality is usually good; hence, 4-6 treatments are basically needed.
Amateur Tattoos
generally these tattoos are very easy to remove, do not usually require multiple lasers, but in few cases, if the ink is perforated at deeper level, a few extra sittings could be needed.
Cosmetic Tattoos
Cosmetic tattoos like eyebrows, eyelash edging, lip line or lip are primarily made of iron-based inks. This can sometimes blaze up and turn black, so an advance test patch should be given.
Non Ablative Skin Resurfacing for Acne Scars and Wrinkles
Q-switched NdYAG laser of 1064 nm provides for a new technology and a better responsive treatment that helps treat wrinkles and scars deeply rooted in the skin. Laser toning is actually being used widely by the dermatologist for resurfacing of skin because it is technique that lay off sufficient energy that punctures in deeply without damaging the top layer of skin, the deep dermis is triggered to produce natural collagen and other essential proteins that make up for healthy, youthful or replenished skin. The procedure is painlessly completed in less than 20 minutes. A patient may have mild redness that will die out within a few hours later. After 3-6 sessions, done at monthly intervals, wrinkles fades away and skin gets firm and smooth. Hence it is also referred to as laser skin toning. It is a better option for improving skin imperfections like wrinkles, acne scars, and stretch marks without complex procedures, risk of damage and extended recovery time and compared to Botox injections etc.
Medium Depth Non Ablative Skin Resurfacing
Wavelength doubled 532 nm, i.e., 1064 Q switched laser is a well-accustomed technology for treating photo-aging. When used at lower level fluences with a bigger spot size, it is a moderate depth laser peel, with lesser downtime and increased patient satisfaction. However, because of the continued risk of post inflammatory pigmented changes in Asian skins, it must be only performed after a patch test and requisite sun protection advised to the patient.
Laser-assisted Hair Reduction
Though the long-pulsed lasers are regarded as the touchstone for the elimination of terminal hair, Q-switched laser has also been attempted with and without topical carbon suspension. Q-switched pulses create a photomechanical effect not just on the tissue but also on hair follicle and hair shaft and, thus, causing decrease as well as delay in hair growth mechanism on face and entire body. Since it is not dependent upon color, it is suitable for all skin types, even on sun burned skins without dreading of pigment changes.
Vascular Lesions
Medlite laser has been seen to be optimum in treating vascular lesions like small spider nevi telangiectasia and cherry angiomas. More than one session could be needed. However, it can sometimes cause purpura which could take up to a week to die away.
Dark Lips
Dark lip is a cosmetic issue; a larger part of Asian skin is concerned about. An approx of 2-sessions of Q-switched 532 nm is an optimal treatment for lip lightening.
There are several complications that are transient reactions of the treatment but do not need discontinuation of treatment. They are as follows:-
- Immediate erythema
- Rebound hyper pigmentation
- Whitening of fine hair
- Physical urticaria
- Acneiform eruption
- Minute petechiae
Then there are some complications that require either termination of the laser treatment or alteration of laser parameters. The complications are as follows:
- Leukoderma
- Severe urticaria
- Severe acneiform eruption
- Mottled hyper or hypo pigmentation
- Herpes simplex activation
Scarring and Ghost shadows can take place with tattoo treatment at higher fluencies.
Pre and Post treatment care?
- To increase the efficacy of laser treatment, combine it with following procedures:
Use of Topical sunscreen - Topical bleaching agents, for example, Kligman’s 7% and arbutin formula
- Intralesional tranexamic acid (5 mg/ml)
- Microdermabrasion
- Long pulsed /Deep pulsed light lasers
- Chemical peels (Glycolic acid or Jessner’s peels)
Laser toning process is excellent once the right treatment framework of criterion is chosen. However in conditions of melasma and freckles, recurrence is likely to take place. The most typical complications shown with the laser treatment of pigmented lesions are hyper or hypo pigmentation. If hypo pigmentation forms, the treatment interval should be doubled and fluence should be decreased. If hypo pigmentation is dotted, this calls for termination of treatment and choosing the medical line of treatment like kojic acid, azelaic acid etc. Refrain from Hydroquinone-consisting preparations. After duration of 2-3 months, once the hypo pigmentation dissolves, laser toning therapy can be resumed at a lower fluence (less than 2.5 J/cm 2). Temporary edema, urticaria and erythema are short term and settle in a few hours.
What to Expect During and After the Procedure
Laser toning and resurfacing is done by a dermatologist or a plastic surgeon, so you are generally in safe hands if your doctor has hands on experience and skills with laser devices. Since it is an outpatient process, you will not have to stay overnight in the clinic.
The dermatologist may treat either treat skin imperfections individually like wrinkles under or around eyes, lip area, frown on the forehead or treat your entire face. For small portions, the dermatologist will numb the part to be treated with a local anesthetic and may also solemn you. The doctor may need to give you a general anesthesia if your entire face is being treated.
Treating only the small areas of the face usually takes about 30 to 45 minutes. A full-face laser toning treatment takes up to 2 hours.
In Follow up with the laser procedure, the dermatologist will bandage the treated areas. After 24 hours, you will be required to clean the treated areas for about 4-5 times a day and then apply a lotion like a petroleum jelly to prevent forming of scabs.
Swelling of the treated and nearby areas after laser skin resurfacing is normal. Your doctor may also recommend steroids to deal with swelling around your eyes. Taking an extra pillow during sleep at night can also ease off swelling.
